Tapestry/ECDOH Surveillance Testing Program

 

Tapestry Charter School has partnered with the Erie County Department of Health (ECDOH) COVID-19 Student Testing Program.  Our joint goal is to return students to an in-person learning experience, while also ensuring the safety of all students, faculty, and staff by reducing the risks of COVID-19 transmission in our school settings.

We HIGHLY ENCOURAGE all parents to participate in this Student Testing Program and give consent for their students to be tested at Tapestry, regardless of vaccination status.  This program is different than our testing program from the 2020/21 school year.  These tests are non-invasive saliva/cheek swab tests.  More information is included in the links and steps below.  Parents and families will need to fill out separate forms for each child they would like to consent to be tested.

Step 1: Review the ECDOH Program Overview Presentation

Step 2: Review the Quadrant Program Registration Video. Quadrant is the laboratory we are partnering with to process and analyze the samples collected. 

Step 3: Complete and submit the ECDOH Program Consent Form

Step 4: Register with and complete the Quadrant Program Consent Form.

Step 5: (Optional) Complete and submit the Abbott BinaxNow (Nasal Swab) Testing Consent Form.
